STANDARD OPERATING PROCEDURE

INTRODUCTION

The document provides guidance and the standard operating procedures (SOP) for COVID HOT HUB to support clinical and non clinical staff in the management of patients with suspected COVID.

OPERATING HOURS / SESSIONS

Opening hours:

  • Monday – Friday
  • 9 am – 5.30 pm

Session timings:

  • 2 sessions / day
  • AM: 9 am – 1 pm (last patient 12.30 pm)
  • PM: 1.30 pm – 5.30 pm (last patient 5 pm)

Slots:

  • 30 minute slots / per patient
  • 16 patients day = 80 appointments / week
  • Patients to be triaged by own GP and booked by own Practice into Hot Hub.
  • No walk ins.

STAFFING

  • The Service is GP Led.
  • Staffing mix will consist of a GP, Receptionist or ANP, Receptionist (with a GP oversight).

ROTA

Rotas will be requested via the Hot Hub Whats App Group.

PPE

  • Own clothes to be worn to work.   A change of clothes is recommended.
  • PPE is provided and this must be worn during your shift:
  • Gloves (single use per patient)
  • White full body suit / Long sleeved gown (sessional use)
  • Plastic apron (single use per patient)
  • Visor / Glasses (sessional use)
  • Fluid resistant surgical mask (single use for clinician/receptionist)
  • PPE should be changed in line and don/dof in line with current guidance.
  • Clinical waste bins are provided for the disposal of PPE at the end of sessional use.
  • Visors can be washed with soap and water and dried with a paper towel and left to dry between patients.

INFECTION CONTROL

Staff

  • Clinicians should be bear below elbow.
  • Long hair should be tied back.
  • Hand gel is available and regular hand washing should be done in line with current guidance.
  • Minimal physical contact with patient.
  • Social distancing 2m rule applies.
  • PPE changed as detailed above.

Ventilation

  • Widows should be opened to allow sufficient ventilation during the session(s).
  • All clinical equipment is to be wiped down using Clinell wipes after use by the receptionist.

ROOMS

  • Work surfaces and desk in clinical rooms totally clear apart from IT and essential clinical equipment.
  • Work surfaces, door handles, desks, keyboards and mobile phone will be wiped down using Clinell wipes after use by the receptionist.
  • Clinical room(s) and patient toilet will be deep cleaned after each am/pm session by the NHS appointed cleaning contractor.
  • The hot hub site will have a full clean at the end of the day by the NHS appointed cleaning contractor.

Clinical Equipment

A range of clinical equipment is available

The following equipment will be used to take patient observations:

  • Sphygmomanometer
  • Pulse oximeter
  • Otoscope
  • Stethoscope
  • Thermometer
  • BM Monitor

Stock check to be completed at the end of each session. Contact Operations Manager for replacement stock.

Oxygen and Nebuliser available on site.   

Contact Operations Manager to arrange replacement of Oxygen.

IT

The Alliance use EMIS consultation.  

After logging into Windows, load EMIS and log in with your EMIS log in details using the Alliance organisation ID 28915. 

  • Task prescriptions / tasks back to the local GP.
  • Receptionist will patient practice for prescription.
  • Do not use ICE.

EMIS Notes / Coding

  • Consultation notes should be completed in line with record keeping principles.
  • For audit purposes (contract requirement) – ensure appropriate COVID read code is used (currently suspected coronavirus infection).
  • If patient DNAs – read code: did not attend – no reason.

GOVERNANCE

A daily profoma monitoring appropriate/inappropriate referrals will be completed at the end of session and emailed to:

CONFIDENTIALITY / HEALTH & SAFETY

Confidentiality

  • Principles of the Data Protection Act (1988) and General Data Protection Regulation (GDPR) 2016 for safeguarding confidentiality of patients accessing the service apply.
  • Staff will be required to sign a declaration of confidentiality prior to first session (in induction pack).

Health and Safety

  • Fire – follow local protocol. This will be explained on induction.
  • Accident – fill in incident form (in staff induction pack) and return to the Operations Manager.
  • Significant Event – fill in form (in staff induction pack) and return to the Operations Manager.
  • Safeguarding – in the event of any safeguarding concerns, take necessary action and escalate to Safeguarding Lead (Dr M Singh).

SECURITY

  • The Security Team will open the main gates, deactivate alarm each morning and set the main doors to exit only.
  • Alison Stone (Team Leader, Stroke Services) will be a designated ‘side’ card holder for the duration of the project. In her absence, Alison will inform Jo of the name of the individual within her team who will pick this up interim.
  • Jo Williams (Operations Manager) is a designated key holder for the building.
  • Building passes will be issued by Jo Williams to staff working on COVID rota and an audit log kept.
  • The Cleaning Team (following deep clean at the end of day) will secure building when they leave (as their normal practice).

IN EVENT OF EMERGENCY

  • Shout for help.
  • Remain 2 meters away at all times.
  • Ensure wearing PPE.
  • Ascertain if an ambulance is needs to be called and do so if needed.
    Communicate:
    • Location
    • Male/female
    • Suspected COVID-19
    • They will ask if the patient is alert/breathing
  • Dial 999.
  • There is oxygen / nebuliser available if needed.
  • Clinician to document any actions taken on EMIS

TIMESHEETS/INVOICING

  • Invoices to be sent to jo.williams34@nhs.net by the 10th of each month.
  • Invoice must be for the month work ie 1 May – 31 May.
  • Payment will be made in arrears, at the end of the month direct to your bank account

COMPLAINTS

Whilst on shift if there is a complaint, take a relevant phone number and email/ring Jo Williams, Operations Manager (jo.williams34@nhs.net) with brief details.   Jo will call the individual to discuss their concerns.
